Remote monitoring system

The remote patient monitoring system consists of three synchronized elements:

• remote monitoring measuring devices at the patients' home
• an electronic patient record specifically developed for chronic heart failure which all measured vital signs data are stored in automatically via mobile phone
• two telemedical centers (TMC) based at the Charité Berlin and at the Robert-Bosch-Krankenhaus Stuttgart, where medical personnel daily assess the recorded data based on each patient’s medical history.
